MAYDAY Recruitment is currently seeking an experienced Home Care Coordinator to join a well-established and purpose-driven organisation within the aged care and community services sector.

The Role

As the Home Care Coordinator, you will be responsible for coordinating services across Commonwealth Home Support Program (CHSP) and Veterans’ Home Care (VHC) clients, ensuring high quality service delivery, compliance and positive client outcomes.

Role Details
๐Ÿ“ Location: Blacktown Area | Fully Onsite
๐Ÿ•˜ Hours: Monday–Friday | Full Time Hours
๐Ÿ’ฐ Rate: $45/hr + Super
๐Ÿ“… Start: ASAP
โณ Contract: Temporary Contract with potential for extension

Key Responsibilities
• Coordinating onboarding and reviews for CHSP & VHC clients
• Completing phone-based assessments and developing care plans
• Managing service agreements, documentation and data entry
• Liaising with clients, families and internal stakeholders
• Managing incidents, complaints and escalations
• Referring clients for additional assessments and support services
• Ensuring compliance across care plans, documentation and reporting
• Supporting audits and continuous improvement initiatives

Ideal Candidate
• Previous experience within Home Care, Aged Care or Community Services
• Experience working across CHSP, HCP, VHC or My Aged Care highly regarded
• Strong coordination and administrative capability
• Excellent communication and stakeholder engagement skills
• Highly organised with strong attention to detail
• Ability to manage competing priorities within a fast-paced environment
• Confident using multiple systems and managing documentation accurately
